  • Family and social history relevant to nutrition care
  • Medicare and private health insurance details
  • NDIS registration number (if applicable)

This information is typically collected directly from you, or with your consent, from other health professionals or referral sources.

  • Provide safe and effective dietetic care
  • Communicate with you and other health professionals involved in your child’s care
  • Maintain accurate clinical records
  • Complete business activities including the process of payments, financial claims and managing appointments
  • Meet legal and professional requirements

Dealing with us anonymously

You have the option to remain anonymous or use a pseudonym when contacting us—for example, when making a general enquiry through our website or over the phone.

However, in most cases, we are unable to provide dietetic services without identifying information. This is because we are legally and professionally required to collect accurate

health and personal details to ensure safe and appropriate care. We will advise you if anonymity is not possible for the service you are requesting.

How we collect your personal information

At Coastal Kids Nutrition, we may collect your personal information in a number of ways. These include:

  • Through our website when you complete our enquiry or online booking form
  • Via phone, email, SMS or when you communicate with us via social media
  • During the completion of your registration form
  • Directly and in person during your nutrition assessment, review(s) or while providing other nutrition services

  • Other healthcare providers including GPs, specialists, allied health professionals, hospitals, community health services, pathology and diagnostic imaging services
  • Your Medicare, private health fund or NDIS

When, why and with whom do we share your personal information?

Your personal and health information is treated with the strictest confidence. At Coastal Kids Nutrition, we will always aim to obtain your consent before sharing information unless we are legally required to disclose it. It may be shared with others only when necessary and in accordance with privacy laws. Your personal information may be shared with:

  • Referring and treating healthcare professionals (e.g. paediatricians, GPs and other allied health professionals) involved in your child’s care, to support a coordinated and collaborative approach
  • Other medical specialists or allied health providers, but only with your informed consent
  • Medicare or private health insurers, when required for billing or rebate purposes /li>
  • IT and administrative service providers (e.g. secure practice management software), who may have access only as part of delivering technical support and who are bound by confidentiality obligations
  • Legal authorities or emergency services, if required or authorised by law, or where there is a serious threat to life, health, or safety, or it is impractical to obtain the patient’s consent

Additional situations where your personal and health information may be shared include:

  • Statutory requirement to lawfully share certain personal information, such as mandatory notification of certain diseases
  • To establish, exercise or defend an equitable claim
  • For the purpose of confidential dispute resolution process

Only people who need to access your information will be able to do so. Other than in the course of providing nutrition services or as otherwise described in this policy, our practice will not share personal information with any third party without your consent.

Our practice will not use your personal information for marketing our services or goods without your expressed consent. If you do consent, you may opt-out at any time by notifying us at Coastal Kids Nutrition in writing.

We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent.
